2-week (±1 week) follow-up after adjustment of therapy or hospitalization. Follow-up visits include brief clinical assessment, serum creatinine, BUN and electrolytes (local lab), and NT-proBNP (local lab biomarker guided arm only). Follow-up visits continue every 2 weeks until therapeutic targets are reached, or therapy is not possible.
